Clicking on Add a Question will open up a side panel that will allow you to specify the following information:

After you have created your survey, you will be redirected to the Edit Form screen. This view allows you to build your survey form.
Create a new survey
To create a new survey, click on the ‘New’ button found on the top navigation bar (dark blue in colour)
Viewing My surveys
Under my post, you can view all the surveys you have created. Surveys are arranged in descending order (most recent first).
Explore public surveys
Under explore, you can browse through public surveys published by other users on the application.
